The American Hospital Association's Community Connections initiative is partnering with national organizations to make relevant publications, tools and resources more widely available to the hospital field. The first three resources, available at www.caringforcommunities.org, help hospitals and health organizations build or implement palliative care programs.
They are: The Case For Hospital Palliative Care, by the Center to Advance Palliative Care; Clinical Practice Guidelines for Palliative Care, by the National Consensus Project For Quality Palliative Care; and a Discussion Guide to Help Clinicians Discuss Treatment Options/Palliative Care with Patients and Families, by the National Hospice and Palliative Care Organization. Community Connections spotlights the many ways in which hospitals serve their communities.
