The Centers for Medicare & Medicaid Services will provide up to $128 million to organizations that partner with nursing facilities to implement evidence-based interventions to reduce avoidable hospitalizations for long-stay residents enrolled in the Medicare and Medicaid programs. Eligible organizations include physician practices, care management organizations, and other public and not-for-profit entities.
Participants in the four-year initiative will collaborate with states and health care providers to facilitate transitions to and from inpatient hospitals and nursing homes; support improved communication and coordination among providers; and coordinate and improve management and monitoring of prescription drugs.
